Errors when attaching screenshots to forms

I do not have a specific error, I wish I did. But in general my team of users reports 5-10% error rate on attaching screenshots in general to forms. We use a lot of them unfortunately for our checklists which creates additional frustrations for the team. My first thought is always lets verify wifi is being used and functional, which we have. I have been giving my team a variance on WW completion rates in general to compensate but it is not ideal. Thanks for your time!
